  1. Fuel oil or Lubrication Oil?

    Either is unlikely due to constant monitoring.

    But how?....sudden large leak.

    What happens? Engines stopped....immediate remedial action taken.

It's possible, but highly unlikely since there is no economic reason to run out of fuel.  Depending on the size of the ship, it may take a few tug boats to bring it safely back to port.  At about a few thousand an hour it will be quite an expensive project. It would be much cheaper to ensure adequate fuel supplies are aboard.
