  1. Everywhere in the world ,  NO!

In 2006, the United States had a Henley Visa Restrictions Index of 130. This means that holders of regular U.S. passports had visa-free entries into or were entitled to obtain visas on arrival in 130 countries and territories. The United States, Finland, and Denmark ranked first on the list.[44]

For travel on a diplomatic passport or on an official passport issued by a foreign country, a visa prior to travel will likely be required by a destination country, even if the destination country allows visa-free entries by or issues visas on arrival to holders of regular passports issued by the same foreign country.

Some countries that require a visa with a US passport include

    Brazil

    Russia

    China

    Most of the middle east

    India

    Pakistan

    Most of Africa

  3. No, the USA has what's called a "reciprocal" policy. That means that for countries that the USA demands a visa for its citizens to visit here, that country requires a visa for U.S. citizens.

I know Brazil, at least, requires a visa.

  4. No. It varies from country to country. You may be able to go to most countries in Europe without having a visa but only as a tourist. For other places you will normally be able to get a visa when you arrive but it's always best to arrange one in advance.
