  1. If you have some shoddy dental work then yes it is possible to have some air trapped under fillings and caps. It is rare though. If you are new to diving then it could be a possibility. If you've been diving for a while I would tend to lean towards other causes such as a mild DCS. The pain in your teeth could very well give you severe headache. You should get checked out by a doctor and dentist.

  2. Air in your teeth would not cause a migraine.  Migraines are a very specific type of disorder.  If you have referred head pain from your teeth, or a headache after scuba diving your should be evaluated.

  3. No, air cannot get trapped in your teeth.  If you're feeling pain in your upper teeth, it's referred pain from your frontal sinuses (which can get stopped up - it's called Sinus Squeeze).

In any case, pain in your teeth or sinus pain is not the cause of migraine.  Migraine is caused by enlarged blood vessels pressing on nerves in the brain.
