According to R. A. Salvatore, author of Icewind Dale; orcs and trolls are too stupid to ride a horse.

The horse can sense stupidity and will not allow them to mount.

For faction quest, pretty much turning in Cloth etc, some epic mounts are drops from high level instances. Some are from the newer factions.  Dragons, etc..  You have to be revered to get a different mount for most if not all factions.
