  1. well  the best city which fits the (medieval period)  and the location u required  will be Alexandria --  Egypt  ,,, I t return back to  334 bc  till now  so , it was there during  the middle age period (medieval )  BETWEEN 500AD  AND 1500 AD  ..

  2. This would be medieval London.  Sewage thrown from buckets into a trench nightly on all city streets.  Carcases of dead animal remains from butchers sharing those trenches.  Smoke from numerous chimneys reducing visibility and threatening to choke you.
