Can anybody read what this record says?

I know that It says Ike Gelbert, his wife Sarah and his Bakery but I can't make out anything else.

He was born 16 JUL 1878 and lived at 1626 Lexington Ave., New York.

The registration was dated 12 SEP 1918

It's not your computer screen being bad, it's the fact that the World War I draft cards are so old that they are naturally hard to read.  :) Anyway, it says:

Sarah Gelbert at same address is next of kin

He was medium weight and build, with dark brown hair.. cannot read the eye color.
