  1. Taking away their remote in their room because they ask for help with their TV too many times.  They will just continue to ask despite you taking away the remote.  You're removing an aversive stimulus.  That's just an example.  There could be many other ones.

  2. Easy one:  When you argue with a patient. You are negatively reinforcing the patient's need for attention by (the pt.) being grouchy. Now every time the patient craves conversation or attention, he'll get grouchy, anticipating an arguement from the clinician.

  3. May be incompetent doctors - diagnostics do this-

f.e. prescribing different and somehow opposite medicaments to be taken together?

  4. Sorry, can't think of a thing

  5. I'm not sure I understand the question.

I've heard that a rubber band around the wrist can be used, When you do something you know you're not supposed to, like going to the refrigerator when one is on a diet, you snap yourself. The problem is that you have to be aware of your own failings and want to do something about them.

An alternative would be to place one of those "electronic fences" around the refrigerator and to wear one of those dog collars that zapped you if you tried to pass the fence. But I think most would balk at that one.
