DIYnetwork.com and htgv.com both offer several sets of instructions on this. Just do a search on their homepage - you'll also get good tips for how to hang it on the wall or attach it to your bedframe, plus quantities of fabric to buy, etc.

When you make the frame, drill holes where you want the buttons to go. After putting on the material, take a long needle, or nail or something similar and put that through the back of the hole towards the front. (You should not upholster the back until after this step, just the front.) If you use a threaded heavy upholstery needle that you can put it through the button and sew it to the back where you will have something to thread it through that is larger than the hole in the plywood.

Also if you don't want the buttons facing all different angles counter sink the front side of the holes in the plywood you build the head board out of.
