Most schools require a certified birth certificate, immunization record and 2-3 proofs of recidency.  For specifics you will want to call the office at the school and they will be happy to give you the details.  Many schools will let you pick up the paperwork to fill out at home and bring back with the other required papers.  Oh yes, and try really hard not to cry on the first day your baby starts real school (or at least not in front of him/her)
