  1. You would probably have to be part of a larger program in most cases.  The problem is that there are dangers involved in most lab research, and the university would be concerned about liability if something were to happen to someone who was not a student or employee of that university, especially a minor.

I take high school students into my biology (genetics) research lab all of the time and even have entire high school classes participate.  Our college also does high school workshops every summer.  The key tends to be to get a mentor who cares about education and community service.  In general, this means that approaching an Ivy League or large state school and getting into a lab is unlikely.  Lots of small liberal arts colleges do good research and love to interact on the high school level.  Generally, either contact the departments chair or the individual.  However, contact them after checking out what they do show you can show your level of interest.

  3. Most universities will allow high school students to get some experience with them. Look up some scientists in fields you're interested in, and call them/email them. Prepare a resume to show them that you're capable of working in a lab.

    They might arrange an interview with you, so be prepared for that too- read up on the scientist's papers, and have a few questions of your own to ask about their research.

    Many universities also have internship programs for high school students. This depends on where you live. The National Institute of Health has a really good internship program, but you still really need to email scientists to be your mentor.

    If your parents know a scientist that could take you, that'd be even better.

    Most universities you can sidestep all the bureaucracy, and get straight to working for your mentor. Some institutes require you to fill out forms and take training courses.
