    Perhaps you are reading too many embellished horror stories here on YA....I can assure you that in the hands of a practiced dentist it is a simple and painless procedure.  

    Yes, you can request it, and it will add quite a bit of cost, and additional risk (that is why it is expensive) but today's dentistry is virtually painless.

  3. Yes.  Make sure your dentist has training in dental anesthesia, but he can put you to sleep using IV meds, or nitrous oxide.
