yes they can. a rail could be skewed, or the train is going too fast. also, some people try to put coins and objects on the rails: at high speed, the train could flip, or careen off the tracks!

Hi trains can derail through a number of causes.heat and cold cause rails to expand and contract causing rail breaks but don't normally cause derailments that I know of and the tracks are checked according to rail safety laws.The over heating of a wheel bearing that I've seen do causes most of the derailments  that I've attended to.This is because of unseen damage within a roller bearing.Wayside detectors do montor temperature of passing wheels but are limited devices within the city limits.

yes. Usually via a broken rail, but the most common is a switch run through. Derailmets rarely happen on trians in passenger service. Most happen in yards where it is easier to have a switch set against you or have tracks in disrepair thats may just fall apart. For the most part, debris don't cause derailments. Coins never would. Trains weigh 400 tons. Most derailments are slow speed because they involve switches or the tracks separtate from a fault while the train is standing still.

Trains end up on the ground very infrequently though and NYC Transit strives with good maintainence to avaoid it.

yes.  they have switches like any other train.  going over a previously run through switch will easily derail that train.  several other things can also derail a train.

Of course they can, and it's happened in the past.  Sometimes the track gets messed up, or the wheels, or there could be something on the track.  Any number of things can cause a train to derail.  However, not all derailments result in death, so don't use this possibility as an excuse not to ride the subway.
