  1. Yes.

    US citizens need either a US passport, passport card, or both a driver's license and US birth certificate if going to Canada by car.  If flying, passport book is required.  

    Permanent residents need a green card.

  4. Hi I am a US citizen and I live in Canada. Right now if you want to enter Canada at a land border you just need to have your birth certificate and photo ID.  If you are coming in by plane or boat then you do have to have a passport.  The land borders will not require a passport for American citizens until June 2009.

    Of course this only applies if you are just coming in for a short visit.  If you plan to stay a lengthy amount of time then you do need a visitor's visa and you can get one at the border when you enter.
