Can someone give me a code for a custom scroll box like this

Question

--i want it to be the exact size of this photo:

--i want the scrollbars to be all white, only the arrows black.

--i want to be able to but custom text under the photo for when i scroll down. (like i can change the font and size and alignment and stuff, if the codes are there i know how to change them)

someone please help me out with this please oh please!
