Can someone please tell me the 7 phases of "the change process" in social work practice?

  1. There are many different models of change.  The classic one is Prochaska, Norcross, DiClemente's five stage model.  I'm not sure which model has 7 phases.

    Precontemplation

    Contemplation

    Determination/Preparation

    Action

    Maintenance

    And the real classic is Lewin's work - unfreezing, change, refreezing.
