Can this vintage cart with some slight rust all over be sanded down, wheels changed to look good???

I don't know about vintage.  It looks like a TV tray with a metal air filter on the top.  

After sanding off the rusty spots, spray paint with a metalic paint.  I'd remove the tacky wheels and put on some swivel ball type rollers.  Then, add another shelf on the bottom rack.  Looks like a cute mini-bar.

Probably! Try naval jelly for the rust, then sand it down and use spray paint for metal surfaces (Rustoleum?). Red would look really cool! Good luck!

I think if you are trying to look vintage then just use steelwool to remove the rust spots and use the cart asis would look better.
