  1. Contact the Canadian High Commission in New Delhi. They will be able to help you.

  2. You have to personally contact the Ministry of External affairs at the following address; make an application for extension of visa; carry with you passport size photo, your passport with the visa on it and you can get hold of the form right there and fill it in and make a request;

    you should have a valid reason, itinerary, return ticket that is valid and proof of sufficient funds and if you had already been staying in India for more than 14 days, proof of having registered are all necessary.

    MHA - Director (F), Jaisalmer House, 26 Mansingh Road, New Delhi - 110011.

    You can download the visa form from the following link;

    it is always good to carry two copies of every document with you and also the originals.

  3. you should contact the business you got your card from

  4. You should go to the Ministry of Home Affairs-Foreigners division.  Below is the main page as well as the page w/ the forms.  It depends what kind of visa you have as to whether you will be able to extend.  You may have to leave the country (go to Nepal or some other close place) and then come back.  It is located not far from the India Gate (maybe a mile?).  Hope that helps.  Email me if you have any further questions.
