  1. It is not necessary for you to file any forms.  Be certain that when you depart, you take the I-94 stub (white card) out of your passport and give it to Immigration at the airport.  Also, save your plane ticket stub and be certain to get your passport stamped when you re-enter your country to verify your departure from the US (for possible future reference).

  2. Buy an airline ticket and go back home, bye! bye!

alas! earning all that U.S. $$$ is very enticing!

    decide: homesickness or greed!

  3. well i don't know too much about it but i do know that there is a form, kind of request, you have to submit to the immigration to cancel any kind of case pending. It does not affect you or your status in this country. I don't know the form or request, but you can do some research. Meanwhile, you leaving and coming in the US all depend on the immigration. Nobody can give a precise answer on that.
