  1. The name and address of the previous owner should be on the front of the title.  The signature of the seller should be the same name as what is on the front of the title as owner. You can use that address and you can date it.  However if who you say is the previous owner, is not the owner that is on the front of the title..then there is a problem. That person has to get the title in his name.  You did not say what state this is, and there are many states that have 2,3 or 4 places on the back of the title to put buyers names.  Those extra places are for dealers only, and can NOT be used in private sales.   Example:  If you are buying the car from John Smith, then John Smith's name has to be the name on the front of the title.  If John Smith sold it to d**k Jones, and d**k Jones is selling it  to you...Then d**k Jones has to get a title in his name before he can sell it to you.  People do not "flip" titles into their name because they don't want to pay tax.  Be very cautious with this type of thing...it is illegal to skip title.  Make sure the person you are buying this car from is the owner of the car.  The name that is on the front of the title is the legal owner.

  2. You should try to get an address and should date it.  You would Probably get by without an address tho
