    I ride a lot of Indian Rail in India when traveling and THG so far have not had any bad luck.

    The main causes in a modern system would be faulty switching equipment or human tampering.

    For most,   maintenance,  human error,  and  freak of nature will suffice.   For a car pulling past a crossing gate,  that's just stupid.

    there a lot of different types of accidents, and I have been involved in a lot of them over the last 35 years LOL.

    cold weather causes rails to break

    hot weather causes rails to expand and buckle

    wheels break

    overheated wheel bearings cause quite a few derailments

    peices fall off cars causing them to derail

    improper train handling can do it

    I have had ice buildup in the grooves of highway crossings cause me to derail

    floods,

    washouts

    vehicles hitting rail cars

    collisions

    there is some vandalism but I think it is quite rare, personally I have never been involved in a vandalilsm or tampering derailment.

    you name it, it can happen no matter how unlikely it may seem

    If you are asking about highway crossing accidents, there is really only one answer

    driver inattentiveness

    every train vehicle colllision could be avoided by drivers paying attention, dont blame it on the crossing protection, why would anyone put their lives in anyone eleses hands when it is so easy to look, each and every time.

  8. My avatar name kinda says it all about my negative RR experiences. I'm an engineer, and I've had things like a broken rail put my train on the ground. Fatigued rails can snap in sub zero weather. Also had an unseen stress crack in a freight car wheel before. One third of the wheel fell off and the car launched itself in to the woods. On private factory property, my engine (at 10 mph) fell down between the rails due to old ties that didn't hold the track guage. For the millions and millions of tons that are transported by rail every year, the safety record is good. And fuel efficiency is good. But accidents happen. They just happen to me more.
