Cell phone won't charge/or hold a charge

Question

Have motorola cell phone and when I plug it in to the charger, it usually beeps indicating the charge is starting. Now, it doesn't beep and remains with the battery icon completely white [no bars], then it takes a few minutes and slowly begins to charge. Sometimes beeps "Low charge".

Doesn't keep the charge very long after being plugged in over night.
