Chelsea Disabled tickets

Do you know how to purchase disabled tickets for upcoming Chelsea game in Stamford Bridge Stadium. Do I have to prove my disability to purchase ticket?

 Disabled tickets are only available to disabled members of Chelsea FC. Rota members will automatically receive tickets when it’s their turn on the rota. Match members can apply 28 days before a game via fax, email or post. Application forms are available online or can be sent out to members who don’t have access to internet or fax.
