  1. It is basically a very, very early miscarriage. You would usually not even know you were pregnant, but now since HPT are more sensitive and can pick up the HCG even before you miss your period, you can test positive if you test early. They are actually very common, I read somewhere that 50-60% of first pregnancies end with an early miscarriage. The woman usually doesn't even know though. There is no way to prevent it from happening, it's when a fertilized egg doesn't implant on the uterine wall, usually because of abnormal chromosomes in the fetus.

  2. A chemical pregnancy occurs when a fertilized egg starts to implant but is aborted before the expected AF, usually due to a flaw in the egg, although if you can't produce progesterone naturally, this can cause it too. So you get your period just before or after is was expected, and in reality it was a pregancy. This actually occurs with 50% of all eggs, but most women aren't aware they were pregnant in the first place, although sometimes you can get a positive test and start your period a few days later. Good luck!
