  1. Yes.  You must satisfy your yearly deductible first before the insurance will start to cover costs.  So the doctors and x-ray labs still need to bill the insurance, so it can be "applied" to your deductible.  They will send a voucher back to the provider stating that the amount is your responsibility.  If you skip the step of submitting it to the ins, they won't know you paid that provider out of pocket and it won't count.  Once your paid-out costs total $4,000, the insurance will pay everything after that.

  2. Yes, it seems like what you have typed is saying that you pay the cost up until the deductible and then the insurance company pays 100% (usually this is where the coinsurance would kick in - typically 80/20 but it appears that this plan does not require you to pay any coinsurance which is a good thing).

Note, that you will not be responsible to pay "full cost" but rather the negotiated lower rate that your insurance company has negotiated with the medical provider for its members.

This is another huge advantage to choosing a large and well known company like a United Healthcare, Aetna, Blue Cross, etc. - because they can negotiate much lower rates for their members. This way even if you still have to pay the costs until you reach your deductible then you will still pay a lower price for services then someone without any insurance at all.

Here is some more information on how to choose the best health insurance plan:

  3. Maybe You should try to google it first ,nonetheless, if you prefer some direct resource ,here might be helpful.http://health-insurance.expert-tip.info/...

  4. It means you pay the annual deductible before the insurance begins paying.

  5. Sounds like it's a $4000 deductible with 100% coverage thereafter.  It also sounds like you'll get an annual physical for $35, but the way you typed it I'm assuming you're office visits are also subject to the deductible.

So basically, other than the physical, you pay for everything up to $4000 and the insurance company will pick up the rest.  But, you should get the network discounts while you're in the deductible.

As long as it's cheap it sounds decent, but be sure to compare it to other companies' offerings because some companies are just out of whack with their rates state to state.
