  1. Call the airline and ask them for a claim form.  They should have a place on their website where you can contact them to ask for a claim form.  Misplaced shipments are common for any high volume shipper.

If they do not help you or give you the run around just call customer service everyday until they give you the information you need.

If for whatever reason you are still not able to get a claim for you can send them an invoice for the value of the goods you shipped.  Be clear and specific if you do this. A description, article count and declared value should suffice.

You will need to understand that the shipper is only liable for a limited amount of money.  This is usually written in fine print on your receipt. If not it may be on their website.

If the value is within their limitations of liability you should be covered.

If you sent a very high value item such as a diamond ring worth thousands of dollars they are only liable up to a certain amount.

If you paid for insurance above their limitations of liability you should be covered up to the limit of the product you sent or the limit of the additional insurance.
