yes, if you neglect that area, not only will you have problems with the usual germs and bacteria, but rust could also form, causing further problems.

Just use a good old toilet brush, a little cleanser, I like Comet, and get to work! Only takes a minute or two.

~~That is a must to be cleaned! You can make a paste of baking soda and vinegar ( it will bubble-that's normal) and use an old tooth brush to apply or buy a commercial cleaner. Then you will need a good toilet brush, and after letting the solution you choose to use-sit 10 minutes, just give it a really good scrubbing! You'll feel better, and your bathroom will smell much nicer!~~

the brush will not do a good job,  wear rubber gloves and with a sponge and some cleanser clean under the rim.

yes use clorox toilet bowl cleaner it has a special shape and nozzle to clean under the rim

Invest in a good quality toilet brush, cheap ones scratch the bowl

Yes you should.

Squirt the cleaner under there and wait 10 mins.

Then clean under there with a toilet brush that has the extra rim cleaner attached.

If you don't have the brush wear gloves and use toilet paper wrapped around your hand and rub under there. Less pleasant than the brush but will do the trick.
