  4. The first sentence is correct.  The second sentence implies that the lady is not as dirty as another lady because "cleaner" sounds like an adjective.  The second sentence would be correct without "lady" because "cleaner" as a noun describes a person who cleans.  

  5. 'The cleaning lady' or simply 'the cleaner'.

    The lady who cleans is working on the second floor.
