Creating a Training Document for my Manager.?

I’m trying to create a measuring tool for new employees. I give a 4 week training class and my manager wants me to create a document that shows how well they do in training. Did they understand the materials - weaknesses and strengths? Does anyone have any ideas of where I should start?

  1. Come up with a list of what it is they're supposed to be learning.  Then figure out a way to measure whether or not they've learned each skill.  

    Once you actually know what it is you're measuring, then you can move on to making the document.  If you want get fancy and assign points to each skill level (1 = dumber than doorknob...5=mastered skill) and stick it in a spreadsheet that adds everything up and gives them a score.  Or just make a word doc that is their evaluation sheet - list a skill - have checkboxes for score and probably a few lines for comments.
