Credit card balance transfer fee?

If I transfer a balance from one card to another, and the new card has a transfer fee of $100, will I have to pay all of the $100 on my first statement, or will they just add it to my overall balance. Also, if the new card has 0% apr on purchases for 6 mos. and 0% apr on balance transfers for 12 mos. and I'm transferring $4000 what can I expect my minimum monthly payment to be for that first 12 months with no interest?

Usually transfer fees run 3% of the amount transferred to a max. of say, $50?  $100 sounds high...anyway they will just add that fee to your total balance and you will pay on it monthly. On $ 4000, depending on the company, I would expect a payment in the $150. - $200, range.
