Davey Jones. Tentacles. Ewww. There's just something inherently evil about an octopus. The heart in the box thing is just disturbing as well.

The vampire from "The Forsaken". Not in the scary way, but the creepy, dont want to meet him in a dark alley kind of way.

Freddy Krueger. Everybody has to sleep, and that's where he gets you.
