DOES THE D.A. HAVE TO SERVE U A SUBPEONA FOR "EVERY" COURT DATE THAT THEY NEED A WITNESS TO APPEAR..... OR IS THE WITNESS SUPPOSE TO KNOW WHEN TO APPEAR?

They serve a Subpoena to the last known address of a witness; if the witness fails to notify them of their new address, the witness can be held in Contempt of Court.
