Declaration of residency in california and notarized guardianship papers?

I’m registering into high school and it says in the requirements that if I’m living with another family member I need to complete a Declaration of residency…how do I get that? (I'm living with my uncle)

I also need to have notarized guardianship papers...where can i get that??!

  1. You can get the Declaration of Residency form from the school district. Just as an example, here.... http://www.pvpusd.k12.ca.us/schools/docu...  is one from a typical CA school district.

    The guardianship papers your Uncle or parents can give you a copy of - your parents need to have signed them already to make it legal for your uncle to act as your guardian.
