Sorry - this isn't the site for people to do your research work or fill forms or do homework for other people. If this were a genuine question, they would have said "Could anybody please describe...."! and they wouldn't have repeated it unnecessarily as so often happens in a copy-and-paste job.

ANSWER:

I was once part of a team of two. The leader was an electrician and I was his mate. We worked as a team and achieved a very positive outcome.

I held the ladder whilst he went up it and changed a light bulb.
