  1. i have heard of children being injured by parents picking them up improperly - such as grabbing them by the arm and lifting them, hurting their shoulders unintentionally.  i suppose the factors here might be lack of attention (maybe the parent has three kids running around and inadvertently grabs one in the wrong way to pick him up), or lack of knowledge (maybe that's how everyone in their family picks up children).

    also i guess you could say time factors for parents could add up to unintentional injuries among children.  parents who work late hours, leaving children to their own devices (i'm thinking older children here, middle or high school) after school.  kids can get some goofy ideas of "fun" things to do around the house.  and when teenagers start driving, that's where the unitentional injuries multiply.  parents have a limited amount of control when a child is on her own behind the wheel.

  2. 1. Adults not supervising properly. This includes chatting to each other on the playground instead of interacting with the children, gathering at one end of the room while the kids are playing, and getting so involved in cleaning up and getting ready for the next activity they forget the kids,

    2. Not inspecting the room and playground daily for safety hazards. Did someone forget to put the safety plug back in the outlet ? Did a plastic toy crack, leaving a sharp edge? Did neighborhood kids break off a piece of the playground equipment last night, leaving splinters? There should be a checklist.

    3. Not having reasonable rules for the kids to follow that they understand and, ideally, had a hand in developing. There need to be clear expectations and a procedure for dealing with kids who cannot comply. (That's a whole 'nother question!)

  3. Being on the computer when you should be watching your kids........Not paying attention to the kids in your care.
