I assume that you are referring to the incident in which Vince McMahon was squashed by the metal sign after a RAW episode featuring his million dollar mania.

Even though it was made as if it was a legitimate accident, with all wrestlers breaking their kayfabe at that time by helping Vince even though some of them such as Shawn Michaels are enemies of him on-screen and with the fact that Vince called out Triple H by his real name at that time, the incident itself was kayfabe.

I think it was a way to put an end to his million dollar mania segment, and to pick where it left off when he had another death storyline when his limousine blew up, but he had to scrap that storyline due to Benoit's death in which Vince suddenly appeared on television to talk about Benoit's death when he was supposed to be dead.

This is my take of this.
