If you mean when Aizen and her got in a fight then no she didn't die she just passed out and went unconscious then I guess while she was recovering they never showed her but later in the series they showed her in the soul society while Toushiro was in the real world and Momo talked to Toushiro on a wireless connection. So... she is alive.

No, Momo is alive. You must have seen the part when Aizen stabbed her rite? Well, she's okay and alive...but mentally distressed. She still can't accept the fact that her beloved captain Aizen betrayed her and Soul Society and still believes that he has reasons why he betrayed Soul Society.

You'll be able to see her again in the Arrancar Arc, don't worry. =)

No, she was healed in later episodes.  But, she had a mental shock and is currently moved out of the picture.
