  1. No.   Health insurance doesn't cover dentists, anyway.


  2. No. PCP usually refers you to medical specialists, but not to dentist. For a dentist, you just have to call their office directly for an appointment.

  3. Present the card when you arrive for the appointment, before you get the work done, not after.
