  1. Yes you will need a visa to enter Bolivia.  The only South American nations that requires visas for American citizens for any reason are Bolivia, Brazil, Paraguay and Suriname. Before 2006, U.S. citizens didn't need visas, however the new government under Evo Morales has implemented the new visa requirements since the U.S. requires Bolivians to have visas to enter the U.S.


  2. Yes. Its the only identification document that will be valid outside of the US. You can get it done at the post office.

  3. You will need a visa and it will cost you $100.  To get the visa you will need proof of yellow fever vaccination, a photo (at least here in the U.S.), a ticket leaving the country (could be a round trip bus ticket) and a hotel reservation or invitation from a Bolivian resident.
