  1. I suggest you to be in contact with person from the Peruvian Embassy in your state in United State to confirm if you need a VISA. I think that yes you would need a visa but confirm friend.

    After that you would be Welcome to Perú

  2. You'll get a visa upon entry between 3o to 90 days, you can renew it three times, one month each before you leave the country.

  3. Not a visa...but a US Passport.
