    here is the embassy of Italy in the US, go to their home page and look for the consulates in the US ,look either under visa or consulate, it will be listed there , find the consulate nearest you .

  2. REQUIREMENTS FOR NON-U.S. CITIZENS TO OBTAIN VISA TO ENTER ITALY

    Visa Requirements:

    - - Submit Passport valid at least 3 months beyond the validity date of visa requested

    - - 1 Visa Application form fully completed; to be signed by applicant in the presence of a Consular Officer.

    - - One passport-type photograph

    - - Proof of permanent residence in the U.S. (Green card)

    - - Copy of round trip airline tickets or itinerary, or letter of confirmation from travel agent.

    - - Proof of sufficient funds (Orginal bank statement)

    - - - - If Tourist: Letter from employer stating that the applicant has a full time job and vacation/leave time has been approved.

    - - - - If Business: Letter from employer guaranteeing travel expenses, including name(s) and address(es) of company(ies) to be visited and purpose of travel.
