When you entered Northern Ireland, you would not have needed a visa, and you would have automatically been stamped in by the UK authorities with permission to remain for, generally, 180 days (not certain why you were limited to 90...).  At the end of authorized period, yes, you must leave - extensions are generally only granted for medical treatment.  For more information see below:
