  1. Yes you most definitely do and it has to match your tourist card. Birth certificate isn't enough. They don't usually stamp your passport, but it is needed for entry.

    Every tourist should have a valid passport issued on his or her name and a tourist card (the tourist card is available with the airline at the airport). It is important for you to know that the Canadian passport should be valid for at least one week after the return date. If you are planning to work, do business or study in Cuba you need a visa; please contact Consulate General of Cuba."

    You can buy the tourist visa (tarjeta tourista) at the airport in Cuba when you arrive. The visa for Canadians has a validity

    of 90 days and is extendable for an other 90 days, than you have to leave the country. Sometimes the tourist visa is included in the package offered by the travel operator.
