    Indoor only cats definitely have more issues than ones who have freedom to go outside, because many of their natural behaviours (hunting, climbing and scratching) are thwarted.  In the USA where the majority of cats are kept indoors all the time, there are proportionately more pet shrinks than in the UK.  

    It's only as we learn more about cat behaviour, that we realise they too are prone to stress.
