  1. They can be for secondaries.

    There are two kinds:

    Corner grounded Delta - Where one phase is grounded.

    Used for safety in industrial app.s.

    and

    Center tap Delta - Where a center tap in one coil is grounded to provide both ground reference, and for some limited 120V. circuits from two phases.

    Limited rural high load service use.

  3. No.  That side of the transformer just outputs three phases.  the ground return is just carried over from the earth ground on the input side.
