  1. The appropriate format would include putting "USA" on your letter.  If you forget it the letter will probably get there however it may take longer.  I do not know if our postal people are more aware than those in the US.  Where I worked we once received a letter mailed from Arizona to British Columbia that took 6 months to get here.  It went via Bogota, Columbia in South America.  The people addressing it forgot to put "Canada" on it and sent it to "British Columbia".

    "America" is part of the name of the continent we live on, not the country to the south of us although most people, on hearing that name, will think of the United States.  It is not, however, an official designation for the purposes of mailing a letter or dealing with any legal documents.
