  2. You aren't required to have the yellow fever vaccination for those places, however, I recommend that you get it anyway.  It costs about $80, and it's good for 10 years.  You can get them from the public health authority.

No, yellow fever is not endemic to that part of Brazil.  you would have to go over a thousand miles north into the Amazon to reach a Yellow Fever Zone.  Also, Machu Pichu is too high for tropical disease like Yellow Fever and  Malaria. HOWEVER, if you travel by land to Cuzco from the Iguazu Falls there are plenty of Yellow Fever and Malaria zones in between specifically Santa Cruz and other parts of Bolivia and near Manu National Park in Peru, literally down the hill from Cuzco.

    I didn;t read about Bolivia at first, I thought you had said Iguazu Falls, Brazil.  Lower than 1,200 meters of altitude and you don't need the shot.  If you stay in teh Santa Crus region, you may need it depending on what types of activity outisde the city and if you will be sleeping in the jungle.  You need to figure out your detailed itinerary before deciding if you will be entering a Yellow Fever Zone.  You may need proof of Yellow Fever vaccination if you are coming from the jungle and entering the highlands.
