  1. According to the US Embassy in London.  If you are a citizen of the UK and have a Machine Readable Passport you need only your passport, visa is waived.

  6. No you don't, thanks to the Visa Waiver Program. You can be here for 90 days. The first source tells about the VWP and the second source tells all the countries that you can visit without a visa.

  7. I believe as it stands now:

    If you have travelled to the USA before, then no special visa, on the plane you fill out a visa waiver on a green form which allows a visit of up to 90days.

    If you have never visited th USA, you may need to go to London and have an appointment at the American Embassy there before you travel.
