  1. As someone else said, ideally you'd print your resume on the back of your headshot.

Alternatively, i'd recomend using a glue stick and lightly glueing your resume to the back of your headshot.  Staples can come loose in a stack and staples could cut someone...

  2. You staple your headshot to your resume so that when someone looks at your wonderful headshot they can then easily turn it over to see your resume.  To make it easy...back of headshot should touch back of resume.

And yes to one staple at top center of headshot.

  3. Print the rezz on the back of the headshot.

  4. Ideally, you should print your headshots and have your resume printed on the reverse -- one sheet of paper.

If you're printing them separately, staple the headshot to the back so you can flip it over to read it, as though it was all printed to a single page (you may need several staples, one in each corner).

If you're not up for stapling, a paperclip (headshot in front, paper behind) should do the trick -- there's no rocket science.

Next time you run them off, though, be sure to print your resume on the back, it'll save you some time!

  5. don't use staples at all, use paper clips if needed, not sure about the picture.

  6. There are of course varying opinions here but I can tell you as someone who works in casting that your ONLY options should be either printed on the back, carefully glued( make sure the paper is not wrinkled) or stapled in all four corners. Never paper clip it. When you are dealing with stacks of headshots guaranteed the paper clip will slip off and you will end up in the trash because they no longer know who you are. One staple at the top center or even just in the upper two corners is not great either. It is just neater and easier for the casting directors to handle if your headshot is firmly attached to your resume.

  7.   Obviously various opinions

Standard is 8 x 10, often with the resume sheet cut to the same size, Borders around the 8 x 10 accepting staples at all corners., Picture, as Katy states, blank sides together.
