Do pokemon lose the continuity of their self-identities when they evolve?

Question

When a pokemon evolves from one form to another, does the consciousness of the original pokemon carry over into the evolved form, or does it get a new identity? If the pokemon's identity is not continuous over evolved forms, is evolving a pokemon identical to terminating a life?

The pokemons mind is continuous. You didn't loose your identity when you went from being a child to a Teen, no you didn't, the same goes for pokemon.
