  1. When you purchase the bottle they will leave it. The server should fill your glass for you when it is empty.

  2. if you buy the bottle they are supposed to leave it on the table after they serve you.

  3. If you buy it by the bottle.  Except for Macaroni Grill, even by the glass they leave their house wine at the table and charge you for each glass of what you've consumed.

  4. Only when you buy the bottle, it is left inside a wine chiller on the table. When you buy a glass, it is taken back to the bar or wherever they store them, always keeping them chilled. Untill you request another glass.

  5. This is an etiquette issue as well as trust.  If ordered by the glass, the bartender will usually pour with the glass delivered by the server.  However, if you order a bottle of wine, it should NEVER leave the hosts range of vision until consumed or otherwise indicated BY THE HOST that the party has finished.  Decanting of wine or other manipulation, such as wiping the bottle down, should always be done in front of the host, never behind the bar.

The purpose for this is assuring the host that no one has tampered or substituted the wine or taken an unauthorized taste.  Most restaurants will leave the bottle at the table (in a chilled wine bucket for most whites and sparkling).  Some restaurants will use a side table if decanting or space is a bit tight.  Any decent restaurant should provide a wine coaster to prevent any drips for getting on the table.

Also, this applies for large groups as well.  A side table should be in view of the host for all staging activities.

  6. If you purchase the whole bottle they normally will.  Some times it will be placed on a side tray.  If you are buying by the glass, no they won't leave it.

  7. Not all restaurants practice that; perhaps only during special promotion. Usually, they only place wine promotion cards on the table (folded similar to reserved notice). Placing wine bottle on the the table is a rare practice.

  8. Only if you order the whole bottle.
